- Fill in your enrolment details (first page)
- Fill in your preferred membership plan and your proposed payment method (full payment or direct debit)
- Sign the authorisation section
- Fill in and sign the Ezi Debit form if you are choosing to pay by direct debit
- Fax, post or bring your application back to us and we'll contact you to confirm your application has been processed.
